What can under-eye filler help with?
Under-eye filler adds volume to a hollow between the lower eyelid and cheek, often called a tear trough. In suitable adults, a hyaluronic acid (HA) filler may soften this hollow and the shadow it creates.
Filler does not remove pigment, treat allergies or remove an under-eye fat bag. Darkness caused by visible blood vessels, pigmentation or inflammation needs a different assessment. Fine lines and loose skin may also need another approach.
Thin skin, prominent eye bags, existing filler and a tendency to retain fluid make treatment more complicated. Persistent swelling or folds over the upper cheeks, sometimes called festoons, need separate assessment; adding volume can make puffiness more noticeable.
Who may be suitable, and when is another treatment better?
A doctor should first establish whether loss of volume is the main concern. The shape of the lower eyelid and cheek, skin thickness, existing swelling and previous procedures all matter. Age alone does not determine suitability.
| Main concern | Approach to discuss | Important limitation |
|---|---|---|
| A hollow with shadowing | HA filler in a suitable patient | Results are temporary, and treatment can cause swelling or other complications. |
| Pigmentation, visible vessels, allergy or irritated skin | Assessment and treatment of the cause; selected skin treatments may help | Filler does not treat these causes. Laser suitability depends on the diagnosis and skin type, with appropriate eye protection. |
| Prominent fat bags or substantial loose eyelid skin | Assessment by a doctor experienced in eyelid conditions, including surgical options when appropriate | Filler does not remove fat or excess skin. Surgery has its own risks and recovery period. |
Having an allergy does not automatically mean filler will help or that it is always unsuitable. An allergic condition causing dark circles needs treatment in its own right. Active irritation or swelling should be assessed before considering an elective injection.
Which filler is best for the under-eye area?
There is no single best brand for everyone. The exact model, its approved use and its suitability for your anatomy matter more than the brand’s country of origin or a description such as “soft” or “firm.” Models within one brand are not interchangeable.
Examples with specific U.S. indications for infraorbital hollowing in adults over 21 include Juvéderm Volbella XC, Restylane Eyelight and Belotero Balance (+). These are model-specific U.S. approvals, not confirmation of Thai authorization, local availability or suitability for you.
The manufacturer’s instructions for TEOSYAL PureSense Redensity 2 describe use around the eyes but exclude under-eye treatment in people with fat-related eye bags, lymphatic fluid retention or markedly loose lower eyelids. A product designed for this area still has important limits.
For treatment in Thailand, ask to see the exact product name, Thai labeling, relevant Thai FDA authorization number, importer, lot number and expiry date. The doctor should explain whether the intended use matches that product’s authorized instructions.
What should you discuss before treatment?
- Allergies and health conditions: disclose previous severe allergic reactions, reactions to fillers or lidocaine, bleeding disorders and immune-suppressing treatment. Contraindications differ by product.
- Medicines and supplements: some increase bleeding or bruising. Do not stop prescribed medicines yourself; discuss any change with the prescribing clinician.
- Previous procedures: tell the doctor about existing filler, fat injections, eyelid surgery and planned skin treatments.
- Reasons to postpone: active skin infection, wounds or inflammation should be treated first. Elective filler is generally deferred during pregnancy or breastfeeding because safety has not been established.
What happens during an under-eye filler appointment?
The appointment should include assessment, a discussion of realistic results and risks, product selection, skin preparation and pain relief when appropriate. A qualified doctor performs the injection and explains aftercare and follow-up.
The amount needed is individual. A package size is not a target that must be injected under the eyes, and more filler is not necessarily better. Do not use an online dose or injection map to plan treatment.
Pressure, stinging or discomfort can occur despite numbing. Both blunt cannulas and needles have limitations; a cannula does not eliminate the risk of a blocked blood vessel. Appointment length varies, and a quick procedure does not mean an immediate final result or no recovery.
When will you see results, and how long do they last?
A hollow may look fuller immediately, but swelling can distort the early result. Bruising and swelling often improve over days to weeks. Attend the agreed review before deciding whether more treatment is needed, and contact the clinic if symptoms worsen or persist.
Results are temporary and vary with the product and individual response. Studies of Volbella XC and Restylane Eyelight followed improvement for up to 12 months, with optional touch-up treatment after the initial session. This is not a guaranteed duration or proof that all filler disappears by a year.
Repeat treatment should follow reassessment of the hollow, swelling and any remaining filler. There is no automatic top-up schedule for everyone. Extra water, avoiding facial movements or a particular lifestyle routine cannot guarantee that filler lasts longer.
What are the risks and warning signs?
Complications can occur even with genuine products and an experienced doctor. Possible problems include:
- Swelling, bruising, redness, pain or tenderness.
- Persistent or delayed puffiness, uneven contours or lumps.
- Blue-grey discoloration, sometimes called the Tyndall effect; existing pigmentation may also appear more noticeable.
- Infection, inflammation or an allergic reaction.
- Accidental injection into a blood vessel, which can cause tissue damage, blindness or stroke. These uncommon complications can be permanent.
Seek emergency medical care immediately for unusual or severe pain, pale, grey, blue or mottled skin, blurred or double vision, any other change in sight, or stroke-like symptoms during or after treatment. Tell the treating clinic, but do not wait for a routine appointment or a reply before seeking emergency care. Breathing difficulty or a severe allergic reaction also needs emergency help.
New lumps, increasing redness, warmth or prolonged swelling need medical assessment. Hyaluronidase may be used for dissolving HA filler in selected situations, but treatment has risks and may require more than one session. It cannot guarantee complete reversal or recovery from a complication, especially loss of vision.
Aftercare and makeup: what should you do?
- Follow the instructions for the product and your treatment. Do not press, rub or massage the area unless your clinician specifically directs you to.
- Keep makeup off open or irritated injection sites. Ask when it is appropriate to resume it, use clean tools and remove it gently.
- Ask about exercise, alcohol, sun and heat exposure, and the timing of laser or other facial treatments. Recovery advice should reflect your symptoms and the product used.
- If a gentle cool compress is recommended for mild swelling, avoid pressure. A compress is not a substitute for medical assessment of warning signs.
- Keep the follow-up appointment and the clinic’s urgent contact details. Do not assume that delayed swelling or new discoloration is normal settling.
How much does under-eye filler cost in Thailand?
The total depends on the exact model, amount in the agreed treatment plan and services included. Ask whether the quote is per cc, per syringe or for the full treatment, and whether assessment, follow-up, any top-up or treatment of complications costs extra. Confirm the current total and any promotion conditions before proceeding.
Choose a licensed clinic and a doctor with relevant experience. Ask how the clinic checks product authenticity, manages complications and arranges emergency referral. Price, reviews and promises of “no swelling” cannot establish safety.
