Age-related neck changes do not automatically need treatment. If they bother you, the useful question is what improvement is realistic for your anatomy, budget and tolerance for recovery.
Which neck concern are you trying to change?
These descriptions can help you explain your concern at a consultation. They are not a self-diagnosis or a treatment selector.
| What you notice | What needs assessing | Important distinction |
|---|---|---|
| Loose, crepey or hanging skin | Skin elasticity, excess skin and weight changes | A tightening device does not remove excess skin. Marked laxity may warrant a surgical opinion. |
| Horizontal neck lines or rings | Crease depth, skin thickness and accompanying laxity | Selected HA fillers may soften individual lines; filling a crease is different from lifting the neck. |
| Vertical cords that become more visible with movement | Platysma muscle activity and overlying skin | Botulinum toxin may soften suitable muscle bands; it does not remove fat or excess skin. |
| Fullness under the chin | Fat, skin and chin or jaw structure | A double chin is not necessarily a skin-tightening problem. |
Why does the neck change?
Age, inherited characteristics, sun exposure and smoking can contribute to skin changes. Weight loss may also reveal loose neck skin. Horizontal lines can be present in younger adults, so a visible crease is not proof of premature ageing. Phone posture alone cannot explain every type of neck concern. [1,2]
Moisturiser can temporarily make fine, dry-looking lines less noticeable, but a cream cannot remove excess skin. Include exposed neck skin in your sun protection: broad-spectrum SPF 30 or higher, clothing and shade. [1,3]
How do the treatment options differ?
Skin laxity: ultrasound or radiofrequency assessment
Energy-based treatments use heat to encourage tissue remodelling. Improvement is generally gradual, and the result is not equivalent to surgically removing excess skin. [3]
Ultherapy Prime uses focused ultrasound with real-time tissue imaging. XERF uses non-needle radiofrequency. Neck suitability must be assessed against the device instructions and your anatomy; a device name, frequency or advertised treatment depth is not enough to choose a treatment. [4,5]
Neither is a substitute for filler when the aim is to fill a particular crease, or for a muscle-directed treatment when prominent bands are the main concern. If you have substantial excess skin, discuss a surgical assessment rather than assuming repeated device sessions will produce a surgical result. Surgery has its own risks and recovery requirements. [2]
For XERF, ask how treatment-tip lots are checked. A recall covers certain XERF EFFECTOR 60 models and lots distributed in countries including Thailand. It does not cover every XERF device or establish that Smooth Clinic received affected stock. [9]
Horizontal creases: selected HA filler
A small randomised trial supports the use of a specific hyaluronic acid filler for static horizontal neck lines. That finding does not establish that every filler is suitable for the neck. Product choice and its authorised use in Thailand need checking. Read the neck filler guide for the procedure-specific benefits, limits and risks. [6]
Vertical muscle bands: botulinum toxin
Botulinum toxin reduces muscle activity and may temporarily improve suitable platysma bands. It does not treat every cause of a loose-looking neck, and cosmetic neck treatment should not be confused with treatment for neck pain. See the neck Botox guide for more detail. [7]
Mixed concerns: agree on priorities
A combined plan can make sense when each procedure has a separate purpose. Ask what should come first, when the result will be reviewed and what is likely to remain. More procedures are not automatically better. If a skin booster or collagen-stimulating injectable is suggested, ask for its exact name, neck-specific evidence and local indication.
Results, recovery and planning around work
There is no single result timeline or maintenance interval for “neck rejuvenation”. Device treatments rely on tissue changes over time; swelling can affect how an injectable result looks initially. Agree on a review date and compare photographs taken with similar lighting, posture and angles.
Non-surgical does not mean risk-free or guaranteed zero downtime. Allow for possible tenderness, swelling or bruising rather than booking immediately before an important meeting or event. A follow-up plan matters even if you live locally and can return easily.
Safety questions to cover before treatment
Tell the doctor about medication and supplements, allergies, pregnancy or breastfeeding, implanted devices, medical conditions and previous filler, threads or surgery in the area. Mention any swallowing or breathing problems, muscle weakness, or active skin infection. Do not stop prescribed medication on your own.
- Energy treatments can cause pain, redness, swelling and tissue injury. Device-specific risks need explaining; Ultherapy safety information also describes temporary nerve effects. [4]
- Filler can cause lumps, infection and unevenness as well as bruising or swelling. Accidental injection into a blood vessel can cause serious injury. [8]
- Botulinum toxin can cause unwanted weakness, swallowing or voice problems and, potentially, breathing difficulty. [7]
Seek immediate medical care for breathing or swallowing difficulty, or a change in vision after treatment. Severe pain, unusual pale or dusky skin, or blistering needs urgent assessment. Do not wait for a routine review or try to massage the problem away.
A new neck lump, sudden swelling or a rapidly changing neck appearance should be medically assessed before being treated as a cosmetic concern.
Comparing costs in Thailand
Start with the Smooth Clinic price page, then request an itemised quote for your neck treatment. A face package does not necessarily include the neck, and a price per shot, line or syringe does not tell you the total cost.
Ask which device or product will be used, why the proposed quantity is appropriate, how many appointments are expected, and whether assessment, anaesthesia, follow-up or additional treatment is included. For English-language care, confirm consultation and written aftercare arrangements before booking rather than assuming availability.
Questions patients often ask
Should I treat loose neck skin immediately after weight loss?
Not necessarily. Tell the doctor whether your weight is still changing and ask whether reassessment would make sense before committing to a plan. There is no universal waiting period suitable for everyone.
Will Ultherapy Prime or XERF erase neck rings?
Do not expect every horizontal crease to disappear. Skin firmness and the depth of a particular line are different treatment goals. Any additional procedure should depend on the remaining concern and its own benefit–risk assessment.
What if I do not want injections or surgery?
You can discuss skin care and non-needle options while accepting their limits. Choosing no procedure is also reasonable when the expected improvement does not justify the cost or risks for you.
You can ask Smooth Clinic about an assessment and current costs without choosing a treatment in advance. Bring a clear description of your main concern and any records of previous neck procedures.
